SYMPTOM QUESTIONNAIRE
The following is a symptom checklist that we ask patients to complete before their first visit. Because the symptom list for ADHD in the DSM (the diagnostic manual used by psychiatrists) was designed for diagnosing ADHD in children, we find it has limited usefulness in adults. Over the years we have found that the following questions seem to better resonate with adult ADHD patients.
